Job summary

Halcyon Home LLC is looking for a PRN Home Health Occupational Therapist to support our growing geriatric home health population in Bastrop/Cedar Creek, TX, and surrounding areas. The role involves assessing patient needs, evaluating occupational performance, and providing education and counseling to patients and caregivers. Candidates should hold a Bachelor of Science in Occupational Therapy and possess an active state license. We offer excellent per visit rates and team support.

Qualifications

• Must have an active state license as an occupational therapist.
• Minimum of one year of home health experience preferred.
• Reliable transportation and a valid driver's license required.

Responsibilities

• Perform ongoing clinical assessments to determine patient needs.
• Evaluate patients’ occupational performance in self-care and work.
• Document clinical notes and maintain accurate records.
• Provide education and counseling to patients and caregivers.
