Halcyon Home is hiring a PRN Home Health Occupational Therapist to support our geriatric home health population in Bastrop, TX and surrounding areas. We offer excellent per visit rates and strong team support.

Qualifications

• Bachelor of Science in Occupational Therapy required.
• Active state OT license or equivalent as per 42 CFR §484.115(f).
• Minimum one year home health experience preferred.
• CPR certification and good interpersonal skills.
• Familiarity with HomeCare HomeBase software is a plus.

Responsabilités

• Perform ongoing clinical assessments to determine patient needs.
• Evaluate patients’ occupational performance in self-care, work, leisure, and rest.
• Assess and facilitate ROM, strength, coordination, adaptive equipment, and functional mobility.
• Develop and evaluate patient care plans in collaboration with patients, caregivers, and representatives.
• Document clinical notes, phone orders, and other records promptly and accurately.
• Provide education and counseling to patients and caregivers.
• Supervise and guide OT assistants and home health aides as necessary.
• Apply infection control and standard precautions during patient care activities.
• Communicate with physicians and healthcare practitioners involved in the patient’s care.
• Meet continuing education requirements and demonstrate professional growth.
