Job summary

Halcyon Home LLC is seeking a dedicated Home Health Certified Occupational Therapist Assistant to support geriatric patients in Bastrop, TX and surrounding areas. We offer competitive pay and strong team support.

Qualifications

• Graduate of an accredited occupational therapy assistant program with an Associate Degree.
• Current Texas State License to practice OT.
• Minimum one year of home health experience preferred.
• Knowledge or experience with Homecare Homebase EMR is a plus.
• Compassionate, empathetic approach to elderly care.
• Strong communication and interpersonal skills.
• Ability to work independently and manage time effectively.

Responsibilities

• Administers the occupational therapy treatment plan as developed by the OTR.
• Assess and facilitate therapeutic interventions for ROM, strength, coordination, adaptive equipment, and functional mobility.
• Facilitates patients ROM, strength coordination, use of adaptive equipment and environmental modifications functional mobility and safety issues as related to patients ADL performance needs under supervision of the OTR.
• Applies concepts of infection control and universal precautions in coordinating/performing patient care activities to prevent contamination and transmission of disease.
• Accepts clinical assignments that are consistent with education and competence to care for patients.
• Communicate effectively with physicians and healthcare practitioners involved in the patient’s care.
• Participate in the Agency’s QAPI program and in-service training.
• Meet continuing education requirements and demonstrate professional growth and competency.
• Promote the Agency’s philosophy and policies to ensure high-quality care.
