Golden Valley Health Centers in Merced, CA, seeks a PACE Registered Dietitian to support Central Valley PACE participants. The RD evaluates nutrition status, designs care plans, and coordinates with physicians and the healthcare team.

Schedule: Mon–Fri, 8:00am–5:00pm, with one remote day weekly. Compensation: $48–$52 per hour. Requires RD credential, BLS CPR, and valid CA driver’s license; bilingual Spanish preferred. Experience with geriatric populations preferred.

Qualifications

• Knowledge of adult and geriatrics health, nutrition, motivational interviewing and adult learning experience.
• Excellent customer services and interpersonal skills; working knowledge of Microsoft Office; presentation and training skills.
• Ability to teach; analytical thinking; knowledge of clinical dietetics.

Responsabilités

• Performs nutritional assessments and reassessments.
• Provides nutrition education to participants and caregivers.
• Coordinates with physicians and IDT to implement nutrition plans.
• Documents changes in medical records and communicates with team.
