Job summary

Cross Country Search is seeking a Registered Dietitian to join the PACE program in San Francisco. You will provide nutrition assessments, counseling, and individualized care plans to support seniors in aging safely at home and in the community.

The role requires RD credentials, BLS/first aid, and EHR experience. You will collaborate with an interdisciplinary team and may conduct home visits to deliver culturally sensitive care.

Qualifications

• Bachelor's degree in nutrition, dietetics, or related field.
• Current Registered Dietitian (RD/RDN) credential in good standing.
• Current BLS and first aid certification.
• Experience with electronic health records (EHR) and strong computer proficiency.
• Excellent verbal and written communication skills in English.
• Strong assessment, counseling, and care coordination skills.

Responsibilities

• Perform comprehensive nutrition assessments and provide MNT for PACE participants.
• Develop, implement, and monitor individualized nutrition care plans and therapeutic diets.
• Provide nutrition education and counseling to participants, families, and caregivers.
• Create meal plans that align with medical conditions, cultural preferences, and lifestyle needs.
• Document assessments, interventions, and follow-up care in Epic and other EHR systems.
• Collaborate with physicians, nurses, social workers, rehabilitation staff, and other team members.
• Conduct home visits and community-based assessments as needed.
• Coordinate nutrition-related referrals and community resources to support continuity of care.
• Develop educational materials, clinical protocols, and nutrition-related resources.
• Participate in quality improvement initiatives and program development activities.
• Monitor participant progress and adjust care plans to support optimal health outcomes.
