Cross Country Search is seeking a Registered Dietitian to join the PACE program in San Francisco. You will provide nutrition assessments, counseling, and individualized care plans to support seniors in aging safely at home and in the community.

The role requires RD credentials, BLS/first aid, and EHR experience. You will collaborate with an interdisciplinary team and may conduct home visits to deliver culturally sensitive care.

• Develop educational materials, clinical protocols, and nutrition-related resources.
• Participate in quality improvement initiatives and program development activities.

About the Role

We are seeking a compassionate and experienced registered dietitian (RD/RDN) to join our PACE (Program of All-Inclusive Care for the Elderly) team in San Francisco. As a key member of our interdisciplinary care team, you will provide nutrition assessments, counseling, and individualized care plans that help older adults maintain their health, independence, and quality of life while aging safely in their communities.

Essential Responsibilities

• Perform comprehensive nutrition assessments and provide medical nutrition therapy (MNT) for PACE participants.

Preferred

• Master's degree in nutrition, dietetics, or a related field.
• Experience working with geriatric, frail, or medically complex populations.
• At least one year of experience serving older adults.
• Two or more years of experience in nutrition assessment, counseling, and patient education.
• Community health, home health, managed care, or PACE
