Job summary

Majestic Care of South Bend is seeking a Certified Occupational Therapy Assistant (COTA) to support high‑quality rehabilitative care under an OT supervisor. You will implement treatment plans to enhance ADLs, mobility, and independence while collaborating with the care team to ensure resident‑centered therapy and safety.

The role emphasizes geriatric rehab, attention to documentation, HIPAA/OSHA compliance, and effective communication with residents, families, and staff in a SNF environment.

Qualifications

• Associate’s degree from an accredited Occupational Therapy Assistant program.
• Current state licensure as a Certified Occupational Therapy Assistant (COTA).
• 1–2 years of experience in a skilled nursing facility or geriatric care setting preferred.
• Familiarity with PDPM and SNF documentation standards is a plus.

Responsibilities

• Deliver occupational therapy interventions as outlined in the supervising OT's plan.
• Assist residents with ADLs and mobility to improve independence.
• Monitor responses and report changes to the supervising OT.
• Support use of adaptive equipment and provide instruction to residents and caregivers.
• Maintain safe, compliant therapy environment and document sessions accurately.
• Participate in interdisciplinary care team meetings and care planning.
