The RN works under a Primary Care Provider and Clinic Supervisor to manage clinical care, assist procedures, administer medications, and coordinate across care settings.

Qualifications

• Associate’s degree in Nursing from an accredited program.
• Active, unencumbered RN license.
• BLS certification.
• Minimum of one year nursing experience in a clinical setting with frail or elderly population.
• Ability to work independently with minimal supervision.
• Strong organizational and communication skills.
• Comfort working with diverse populations.

Responsibilities

• Provide and document ordered nursing care and procedures in the clinic, including medication management.
• Check vital signs, support wound care, and manage clinic overflow.
• Coordinate care transitions between hospitals, SNFs, home health, and clinic settings.
• Dispense medications during clinic visits and reinforce education.
• Educate patients, families, and caregivers on disease processes and self-care.
• Support clinical staff and assist with procedures as directed.
• Monitor and document immunizations and vaccine tracking.
• Assist with home visit procedures when required.
