Job summary

TEEMA seeks a compassionate Personal Care Specialist to join an interdisciplinary care team. You will assist with ADLs, provide personal care, monitor patient wellbeing, and support individualized care plans across the center, clinic, and patients' homes.

Responsibilities include bathing, grooming, meal support, and safe patient transport. Prior experience with frail or elderly populations is preferred, and CNA certification is required. This role emphasizes patient confidentiality and teamwork.

Qualifications

• Two years of experience in hospital, nursing home, or community-based setting preferred.
• CNA license required.
• At least one year experience with frail or elderly populations preferred. Training provided if not present.

Responsibilities

• Deliver high-quality care as part of an interdisciplinary team and implement personalized care plans.
• Provide personal care including bathing, dressing, grooming, feeding, toileting.
• Facilitate activities aligned with daily calendar to engage patients.
• Assist with meal preparation, service, monitoring and cleanup.
• Maintain a clean, safe environment and document vital signs and health changes.
• Accompany patients to appointments and assist with transportation.
• Maintain accurate documentation in medical records and ensure confidentiality.
• Participate in team meetings and ongoing education.
