Job summary

TheKey of California, LLC is seeking a Geriatric Care Manager to deliver a holistic, client-centered approach for aging in place in the Portland metro area. You will assess and reassess care, coordinate medical decisions, and partner with families to maintain dignity and independence.

Responsibilities include creating care plans, monitoring client status, managing electronic records, and collaborating with physicians, attorneys, and caregivers.

Qualifications

• 5+ years in community, hospital, hospice or home health case management.
• Bachelor’s or master’s degree in a healthcare-related field preferred.
• CMC or CCM certification preferred.
• Experience with aging, dementia, or disabilities.

Responsibilities

• Develop and implement Comprehensive Care Plans.
• Track and document client condition and changes.
• Monitor and report on client status and outcomes.
• Coordinate with families and support systems to meet needs.
• Provide resources and information for informed medical decisions.
• Manage client Electronic Health Records and billing.
• Coordinate resources for client care.
• Interact with physicians, attorneys, fiduciaries, hospice personnel and community staff.
• Maintain referral resources and advocacy.
