Job summary

TheKey is seeking a Geriatric Care Manager to provide holistic, client-centered care for aging individuals in the Portland area. You will conduct assessments, develop care plans, and coordinate medical and support services while respecting client autonomy.

This full-time role requires 5+ years in care management, a healthcare degree, and relevant certifications. Expect travel to client homes, collaboration with families, and detailed documentation in EHRs as part of a comprehensive care approach.

Qualifications

• 5+ years in community, hospital, hospice or home health case management.
• Bachelor’s or Master’s degree in Healthcare Related Field preferred.
• CMC or CCM certification preferred.
• Knowledge of aging, dementia, and disabilities.
• Experience working with families and complex dynamics.

Responsibilities

• Develop and implement Comprehensive Care Plans.
• Track, assess and document observations regarding client condition and care plans.
• Monitor, evaluate and report on client status, actions/interventions, and outcomes.
• Collaborate with formal and informal social support systems and families.
• Guide families to informed medical decisions.
• Manage Electronic Health Records including billing.
• Coordinate resources for client care.
• Interact with physicians, attorneys, fiduciaries, hospice personnel and community staff.
• Create and maintain referral resources and advocacy.
• Perform additional duties as assigned by supervisor.

As a Geriatric Care Manager, you are responsible to provide a holistic, client-centered approach to caring for clients. The Geriatric Care Manager provides assessments and continuous reassessment and manages comprehensive aspects of care including medical and cognitive challenges of aging in a manner that ensures their dignity, independence and wishes are respected.

Essential Duties And Responsibilities

• Development of Comprehensive Care Plans and implementation of Care Plans
• Track, assess and document observations regarding client condition, changes in preferences and problems/opportunities, medical orders, family requests and other information pertinent to Care Plan.
• Monitor, Evaluate and report on Client Status, Actions/Interventions, and Outcomes
• Works with formal and informal social support systems and families to ensure client’s needs are met.
• Provide guidance, resources, and information to enable families and clients to make informed medical decisions.
• Manage Electronic Health records for clients including billing.
• Coordinate resources for client care.
• Interact with other professionals on the client’s behalf this may include, physicians, attorneys, fiduciaries, hospice personnel, and community personnel.
• Create and maintain referral resources, identification, coordination, management and advocacy
• Additional duties and responsibilities as may be assigned by your supervisor

Required Skills, Education And Certifications

• (5) Years’ experience in community, hospital, hospice or home health case management
• Bachelor’s or master’s degree in Healthcare Related Field, preferred
• Certified Care Manager (CMC) or Certified Case Manager (CCM)
• Demonstrated knowledge of aging, dementia, and/or disabilities.
• Experience working with complicated family systems and dynamics.
• Computer Skills: MS Office, Electronic Health Records and Internet Skills
• Excellent customer service and problem- solving skills
• Excellent verbal and writing skills
• Detail-oriented with the ability to keep up with fast paced processes

Physical Requirements

• Ability to lift and carry up to 10-15 pounds
• Ability to sit, stand and walk for prolonged periods of time throughout the work day
• Frequent local travel by personal automobile to client homes required
• Ability to use standard office equipment such as personal computer, laptop, telephone, photocopy machine, etc.
