Trinity Advocacy Group is growing. We're looking for a thoughtful, resourceful Care Manager who can build trusted relationships, navigate complicated situations and help older adults and their families make informed decisions.

What you'll do

• Assess needs and develop proactive, person-centered care plans
• Coordinate healthcare, home-care and community services
• Attend client visits and medical appointments, if needed
• Advocate for clients during transitions, crises and everyday challenges
• Communicate clearly with clients, families and professional partners

You'll have a collaborative team, regular support and the opportunity to develop lasting client relationships, not one-and-done interactions. This position includes local travel and participation in a shared telephonic on-call rotation.

Salary: starting at $70,000 (commensurate with experience)
