Connie Rosenberg & Associates Care Management, Inc., a respected Aging Life Care Management practice in Northern New Jersey, is seeking a licensed nurse or social worker to join our team as an Aging Life Care Manager. This full-time role combines field client visits with work-from-home documentation and planning.

Qualifikationen

• Current professional license as RN, LPN, LSW, or LCSW.
• Experience with geriatrics in healthcare or social services.
• Strong communication, organizational, and problem‑solving skills.
• Ability to work independently and travel in Northern New Jersey.

• Conduct comprehensive assessments of older adults' physical, cognitive, emotional, and social needs
• Develop and implement individualized care plans
• Coordinate healthcare services, community resources, and support systems
• Advocate for clients and help families navigate complex healthcare and aging-related decisions

• Accompany clients to medical appointments when appropriate
• Assist with transitions between home, hospital, rehabilitation, assisted living, and other settings

About the Role

As an Aging Life Care Manager, you will:

Who We're Looking For

Required Qualifications:

• Current professional license as an RN, LPN, LSW, or LCSW
• Experience working with the geriatric population in a healthcare, social service, or related setting
• Strong communication, organizational, and problem-solving skills
• Compassion, resourcefulness, sound professional judgment, and the ability to work independently
• Comfort with technology and electronic documentation
• Must reside in Northern New Jersey
• Valid driver's license and reliable personal vehicle for travel throughout our Northern New Jersey service area

Prior geriatric care management experience is a plus, but not required. We welcome candidates with relevant backgrounds in nursing, social work, home healthcare, hospital discharge planning, rehabilitation, hospice, and other aging-related services.

Compensation & Benefits

• Annual salary: $75,000–$95,000, commensurate with professional credentials, qualifications, and relevant experience. Compensation is negotiable, with consideration given to exceptionally qualified candidates.
