Qualifications

• Associate's or Bachelor's degree in Nursing.
• Active Ohio Registered Nurse license.
• 10+ years of nursing experience in ICU, geriatrics, hospice, home health, case management, or care management.
• Current BLS certification preferred.
• Strong clinical judgment and assessment skills.
• Excellent communication and relationship-building abilities.
• Experience coordinating care with interdisciplinary teams.
• Proficiency with Microsoft Office and EMR systems.
• Ability to handle confidential and sensitive information with professionalism and discretion.
• Passion for helping older adults maintain independence, health, and safety.

Responsibilities

As theRN Care Manager & Patient Advocate, you will serve as a trusted advisor and advocate for older adults and their families. You will use your nursing expertise to help clients navigate healthcare decisions, coordinate care, and maintain their independence, health, and safety. You will:
