Job summary

All Seniors Foundation is seeking a Geriatric Care Manager to oversee and coordinate comprehensive care plans for elderly clients at home in Los Angeles. You will work closely with families, physicians, nurses, social workers, and community organizations to ensure seniors receive the best possible support while maintaining their independence at home.

You will develop and manage care plans that address healthcare, social, and emotional needs, coordinating services across providers and resources.

Qualifications

• Bachelor’s degree in Social Work, Nursing, Gerontology, or related field.
• Experience in care management, home health, or eldercare coordination.
• Strong communication, organizational, and problem-solving abilities.
• Knowledge of local community resources and healthcare systems.
