Active Day in Louisville, KY seeks a Licensed Practical Nurse to provide compassionate nursing care to members under physician orders and emergent needs. You will support staff and families, document vital signs, and ensure compliance with policies and regulatory standards.

The role involves assisting with daily activities, delivering health education, and collaborating with a cross-functional team to enhance member experience and outcomes. LPN license and CPR are required.

Qualifications

• LPN license and CPR certification required.
• Background check and state regulatory requirements may apply.
• Minimum 21 years of age and one year of healthcare experience preferred.
• Ability to lift up to 50 pounds and assist with transfers.

Responsabilités

• Provides nursing care according to plan of care, physician's orders and emergency needs.
• Monitors vital signs and documents responses to treatments and medications, reporting abnormalities to Director of Nursing.
• Assists members with daily activities including hygiene, meals, ambulation, arrivals and departures, and social activities.
• Provides health education and instruction to staff, members and family members.
• Provides emergency care as required.
• May administer medications with appropriate certification and RN supervision.
• Communicates professionally with members, families, co-workers, and referral agencies.
• Participates in staff meetings and cross-functional collaboration to enhance member experience and center goals.
• Complies with policies, safety and regulatory standards.
• Other duties as assigned.
