Job summary

American Health Communities is seeking an RN Case Manager to oversee care coordination for a Medicare Advantage plan. You will visit patients in person or by teleconferencing to ensure appropriate nursing care and collaborate with physicians to address errors or omissions in claims.

Qualifications

• Minimum of 2 years of clinical nursing or geriatric rehabilitation.
• 2 years managed care experience required.
• 3-5 years case management in managed care preferred.

Responsibilities

• Complete Health Risk Assessments for members.
• Initiate and revise care plans as needed.
• Coordinate with interdisciplinary team to plan and evaluate patient care.
• Educate members and staff on health access options.
• Maintain a caseload and support care coordination activities.
