American Health Partners is seeking an RN Case Manager to oversee care coordination for members enrolled in a Medicare Advantage plan. This role involves in-person and telephonic patient visits, collaborating with physicians to address claims issues, and coordinating with a healthcare team to plan, implement, and evaluate patient care plans.

Qualifications

• Graduate of an accredited RN program.
• Bachelor's degree preferred.
• Current RN license in the assigned state.
• CCM certification preferred.
• CPR for Healthcare Professionals.

Responsibilities

• Complete Health Risk Assessments for assigned members.
• Initiate and revise care plans as needed.
• Maintain an assigned caseload and coordinate with the health team.
• Educate patients and families using available teaching resources.
• Coordinate outpatient discharge planning based on patient needs.
• Participate in audits and prepare reports using MS Office.
