Qualifications

• Must have Master’s degree in nursing and APRN licensure seeking Family Nurse Practitioner (FNP) or Adult Nurse Practitioner (ANP).
• Minimum 3 years general nursing experience; 1 year with frail/elderly population.
• Current CPR certification and active APRN license in good standing.

Responsibilities

• Provide direct participant care and evaluate outcomes.
• Develop and update participant-centered plan of care with physician collaboration.
• Consult with nurses and maintain professional knowledge and standards.
• Participate in quality improvement and administrative tasks.
• Manage multiple priorities while communicating with patients and families.
