Hospice of the Bluegrass Inc is seeking a PACE Nurse Practitioner to assess, initiate, and coordinate direct care for CGPP participants. You will work closely with the IDT and medical providers across clinic, home, and facility settings to ensure comprehensive care.

The role includes education, outreach, and community leadership within the service area. The ideal candidate has KY advanced nursing licensure (or eligibility), at least one year of full-time practice with frail elderly populations,

Qualifications

• Licensure (or eligibility) for advanced nursing practice in KY, including DEA and controlled substance permits.
• Minimum of one year experience in full-time advanced nursing practice.
• Knowledge of the physical, mental, and social needs of frail older adults.
• Minimum 1 year of experience with frail/elderly populations or training upon hire.
• Must maintain Basic Life Support certification.
• Flexibility to travel within service area and provide infrequent home visits.
• Proficiency with technology-based tools, including EMR.
• Ability to work independently and as part of a team with a high level of autonomy.

Responsabilités

• The PACE Nurse Practitioner assesses, initiates, delivers and coordinates direct care for CGPP participants.
• May see participants in clinic/day center or at homes, LTC, or hospitals as needed.
• Coordinates with the IDT and other medical providers; participates in education, outreach and leadership activities.
