The Appalachian Agency For Senior Citizens, Inc. is seeking a Nurse Practitioner to manage PACE participants, delivering primary health care and case management focused on prevention, wellness, and treatment of chronic and acute illness in collaboration with primary care physicians.

You will work with the Medical Director to coordinate the 24/7 PACE Model of Care, perform comprehensive assessments, order tests, interpret results, and prescribe within licensure.

Qualifications

• Graduate of accredited nurse practitioner program.
• Virginia license and current registration to practice as a nurse practitioner; geriatrics/Adult/Family certification preferred.
• Minimum of 1 year experience with frail or elderly population.
• Compliance with State or Federal requirements for direct patient care staff.
• Immunizations up-to-date and medically cleared.

Responsabilités

• Performs health histories and physical exams.
• Orders and interprets diagnostic procedures and labs.
• Prescribes medications within scope of licensure.
• Provides comprehensive, coordinated care for PACE participants in various settings.
• Collaborates with Medical Director and primary care physicians.
• Coordinates and schedules referrals and communicates with care teams.
• Educates participants and trains family/caregivers.
• Maintains confidentiality and supports staff development.
