Job summary

Centene Corporation in Georgia is seeking a Registered Nurse with OB experience to develop, assess, and facilitate complex care management for members with primarily physical needs, ensuring high quality, cost-effective outcomes.

You will evaluate needs, develop care plans, coordinate with providers, and educate members and families while ensuring compliance with state and payer regulations. This role requires GA residency, nursing licensure, and 2–4 years of relevant experience.

Qualifications

• RN with state licensure required.
• 2–4 years of related nursing experience.
• Must reside in Georgia and have obstetrics (OB) experience.

Responsibilities

• Evaluates member needs and barriers to care; plans and coordinates care.
• Develops and updates individualized care/service plans with providers.
• Identifies barriers to care and intervenes to ensure appropriate care.
• Coordinates between member/family and care team for person-centered care.
• Provides ongoing follow-up, monitors progress, and revises plans as needed.
• Educates members and families on diseases, care options, and benefits.
