Job summary

Visions Case Management NM seeks a Service Provider to strengthen New Mexico communities by enhancing the lives of those we serve and work with. You will foster a supportive environment, deliver exceptional services, and empower everyone you interact with.

Responsibilities include monthly consumer contact with timely documentation, drafting annual plans and budgets, maintaining client files, updating tracking systems (Google Sheets), and participating in staff meetings with supervision and

Qualifications

• Four year degree in Nursing, Psychology, Social Work and 1 year of related experience.
• Any four year degree with 6 years of related experience.
• Travel within the entire state of New Mexico.

• Maintain complete and current client case file.
• Utilize and update internal tracking systems 2 x weekly and ensure information is current and complete (Google Sheets).
• Participate in monthly staff meetings and either meet with your regional lead weekly or program manager monthly for supervision and training.
• Respond to internal and external communications within 24 hours.
