As a Service Provider at Visions Case Management & Consulting, you will be strengthening the communities of New Mexico by enhancing the lives of those we serve and work with. Qualifications

• Four year degree in Nursing, Psychology, or Social Work with 1 year of related experience.
• Any four-year degree with 6 years of related experience.
• Travel within an entire state of New Mexico.

Responsibilities

• Facilitate monthly contact with consumers and submit related documentation in a timely manner.
• Draft and submit annual plans/budgets according to program timelines and revise as needed.
• Maintain complete and current client case file.
• Utilize and update internal tracking systems 2 x weekly, especially Google Sheets.
• Participate in monthly staff meetings and coordinate supervision with regional lead or program manager.
• Respond to internal and external communications within 24 hours.

• Facilitate monthly contact with consumers and submit related documentation in a timely manner.

• Maintain complete and current client case file.
• Utilize and update internal tracking systems 2 x weekly and ensure information is current and complete. Specifically google sheets or spreadsheets in general
• Participate in monthly staff meetings and either meet with your regional lead weekly or program manager monthly for supervision and training.
• Respond to internal and external communications within 24 hours.

Required Qualifications:

• Four Year Degree in Nursing, Psychology, Social Work and 1 year of related experience
• Any Four Year Degree and 6 years of related experience
• Travel within an entire state of New Mexico
