Job summary

Visions Case Management & Consulting is seeking an organized, detail-oriented service provider to strengthen communities in New Mexico. You will maintain client files, manage monthly consumer contacts, and support program planning with timely documentation.

The role requires a Four Year Degree in a relevant field or equivalent experience, and travel across the state. The ideal candidate will excel in communication, time management, and problem solving, with comfort using Google Drive and Adobe.

Qualifications

• Four Year Degree in Nursing, Psychology, Social Work and 1 year of related experience.
• Any Four Year Degree and 6 years of related experience.
• Travel within an entire designated region of the state.

Responsibilities

• Facilitate monthly contact with consumers and submit related documentation in a timely manner.
• Draft and submit annual plans/budgets according to program timelines and revise as needed.
• Maintain complete and current client case file.
• Utilize and update internal tracking systems 2x weekly and ensure information is current and complete; specifically google sheets or spreadsheets in general.
• Participate in monthly staff meetings and either meet with your regional lead weekly or program manager monthly for supervision and training.
• Respond to internal and external communications within 24 hours.
