Job summary

Fairview Health Services in Princeton, MN is seeking an RN Care Coordinator for Northland Medical Center. This 0.8 FTE salaried role focuses on gastroenterology, providing proactive disease management, care coordination, and patient education. Monday–Friday, 8:00 AM–4:30 PM with one weekday off.

The RN coordinates care across the health continuum, serving as a resource for patients and staff, and supporting referrals, documentation, and quality improvement initiatives.

Qualifications

• 3 years specialty experience doing care coordination.
• 5 years licensed RN with clinical experience and willingness to learn a new specialty.
• MN Registered Nurse (RN) License.
• Basic Life Support (American Heart Assoc or Red Cross).

Responsibilities

• Accesses, analyzes, plans and provides follow-up care.
• Facilitates referrals and maintains relations with community providers.
• Evaluates patient status and documents progress via calls, results, and reports.
• Provides patient education, medication management and counseling.
• Triages patient concerns and coordinates transitions of care.
• Serves as liaison among patient, family, staff and providers.
• Supports program activities and research efforts as needed.
