Fairview has an immediate opening for a Registered Nurse (RN) Care Coordinator at Northland Medical Center in Princeton. This salaried 0.8 FTE position schedules Monday through Friday from 8:00 AM to 4:30 PM, with one weekday off each week.

Qualifications

• 3 years specialty experience in care coordination or 5 years of licensed RN experience in clinical settings.
• MN RN license
• BLS certification

Responsibilities

• Accesses, analyzes, plans and provides follow-up care.
• Facilitates timely referrals and fosters relations with community providers.
• Evaluates patient status and documents progress through phone calls and test results.
• Provides education, counseling, and medication management related to the specialty.
• Coordinates care transitions across the healthcare continuum.
