Job summary

OnePeak Medical in Oregon is seeking Nurse Practitioners or Physician Assistants to deliver functional, patient-centered primary care across our 20+ clinics. You will provide direct care via office visits and telehealth, with a 4-day work week and no nights or weekends.

Compensation includes a guaranteed salary of 120,000–160,000 with earning potential of 300k+ and relocation assistance. Join a mission-driven practice focused on preventive, functional medicine and collaborative care.

Qualifications

• Active Oregon license as a Nurse Practitioner (FNP or AGNP-PC) or PA-C (or eligibility to obtain)
• Board certification in Family Medicine, Internal Medicine, or relevant specialty
• A passion to learn and practice Functional Medicine
• Strong interest in preventive, functional, and lifestyle medicine
• Excellent communication and patient relationship skills
• Comfortable working in a fast-paced, collaborative environment
• Proficient with electronic health records (NextGen experience a plus)

Responsibilities

• Provide direct patient care through office visits, telehealth, and occasional same-day appointments
• Perform comprehensive assessments, order and interpret labs/imaging, and diagnose acute and chronic conditions
• Develop personalized treatment plans that may include lifestyle interventions, supplements, hormone therapies, and prescriptions
• Educate and empower patients on preventive health and functional medicine principles
• Collaborate with internal health coaches, nutritionists, mental health providers, and clinical support staff
• Document visits in the EHR (NextGen) in a timely and compliant manner
• Participate in team meetings, quality improvement initiatives, and training sessions
• Support clinic operations by contributing to a positive, team-oriented environment
