Job summary
OnePeak Medical is seeking a Functional Health Coach to assess patients, create personalized nutrition plans, and collaborate with licensed providers to optimize health outcomes. Role includes educating patients, coordinating care, and documenting progress in compliant EHR systems.
The position emphasizes root-cause medicine, functional nutrition, and integrated care within our Eugene clinic network. Requires relevant certifications and at least 1 year of experience in nutrition or wellness
Qualifications
- Requires strong ability to interpret nutrition-related data and labs.
- Experience in wellness coaching or similar roles is preferred.
- Knowledge of functional nutrition and integrative health concepts.
Responsibilities
- Conduct nutritional assessments and health histories during visits.
- Develop personalized functional nutrition plans with providers.
- Co-manage patient care and monitor progress during follow-ups.
- Educate patients on functional nutrition principles and lifestyle changes.
- Document care plans and outcomes in EHRs in line with HIPAA.
