Cornerstone Health Center is seeking a Functional Nutrition Coach to support our wellness team in delivering personalized nutrition and wellness care. This role involves patient preparation, lab management, protocol delivery, and follow-up communication to ensure exceptional care and smooth clinic operations.

Qualifications

• Certification in Functional Nutrition Coaching is required.
• Strong passion for functional nutrition and wellness protocols.
• Experience in patient care, lab orders, and medical documentation.
• Ability to manage multiple tasks, stay organized, and maintain attention to detail.
• Excellent communication skills, both verbal and written, with patient interaction.
• Proficient in office software (e.g., Google Drive, ClinicMind) and lab management systems.

Responsibilities

• Prepare prescribed nutrition and lab review protocols for appointments.
• Print and organize resources and documents before patient consultations.
• Pull and prepare supplements based on protocols.
• Assist in ordering labs and align with patient care plans.
• Move and organize nutrition notes and forms into patient folders.
• Set up Zoom and send virtual appointment protocols as needed.
• Meet with patients to review lab results and personalized protocols.
• Discuss nutrition plans and address patient questions and concerns.
• Provide explanations of lab findings and treatment implications.
• Guide patients through dietary and supplement recommendations.
• Document protocol alterations and bill for supplements.
• Export patient files into daily folders and update records.
• Download and order labs with accurate information.
• Handle patient emails and inquiries regarding nutrition and labs.
• Assist with lab audits and track KPIs.

Job Summary:

We are seeking a Functional Nutrition Coach to support our wellness team in delivering personalized nutrition and wellness care. This role involves patient preparation, lab management, protocol delivery, and follow-up communication to ensure exceptional care and smooth clinic operations. The ideal candidate is highly organized, detail-oriented, and passionate about functional nutrition.

Key Responsibilities:

Patient Preparation and Support:

• Prepare prescribed protocols for nutrition and lab review appointments.
• Print and organize all necessary resources and documents prior to patient consultations.
• Pull and prepare supplements for patients based on protocols.
• Assist in ordering labs and ensure lab orders align with patient care plans.
• Move and organize nutrition notes and forms into the patient's daily folder before appointments.
• Set up Zoom and send virtual appointment protocols as needed.

During Appointments:

• Meet with patients to review lab results and personalized protocols.
• Discuss nutrition plans and address patient questions or concerns regarding their health and wellness goals.
• Provide detailed explanations of lab findings and how they relate to the patient's treatment plan.
• Guide patients through dietary and supplemental recommendations, ensuring they understand each aspect of their protocol.
• Answer any immediate patient concerns, clarifying next steps or adjustments needed in their health plan.

Post-Appointment Duties:

• Scan and document any alterations to the patient's protocol, ensuring records are up-to-date.
• Bill and charge for supplements, ensuring all transactions are accurately recorded.
• Finalize documentation and export patient files into daily folders.
• Download and order labs, ensuring accurate information is recorded.
• Handle patient emails and answer in-person or phone queries regarding nutrition and lab protocols.
• Assist with lab audits and track Key Performance Indicators (KPIs) to ensure efficient service delivery.

Lab Orders and Management:

• Manage the lab order process, ensuring accurate payments, tracking, and documentation.
• Work with lab technicians to process and order labs in accordance with patient care plans.
• Ensure all lab results are uploaded to the appropriate folders and patients are notified promptly to schedule reviews.

Lab Review Scheduling and Communication:

• Review and manage the scheduling of lab reviews and consultations, ensuring all relevant patient information is organized.
• Communicate with patients to confirm lab review appointments, align with adjustments, and maintain accurate appointment records.

Qualifications:

• Certification in Functional Nutrition Coaching required(IIN, HCI, CMFP, CNS)
