Job summary

OnePeak Medical Office & Corporate is seeking a Functional Health Coach in Albany, Oregon to apply root-cause nutrition strategies with patients. You will conduct comprehensive assessments, develop personalized plans, and collaborate with medical providers to optimize patient outcomes.

Role requires at least 1 year in nutrition or wellness coaching, high school diploma or equivalent, and relevant certifications. On-site role with benefits including healthcare, retirement plan, PTO, and discounts.

Qualifications

• Strong analytical skills for interpreting labs and root-cause analysis.
• Excellent communication and coaching for client education and motivation.
• Knowledge of biochemistry, nutrigenomics, and holistic health (gut-brain axis).
• Proficiency in nutrition software to track and support nutritional knowledge and support, motivational interviewing, and evidence-based dietary guidelines.
• Interpersonal skills for building trust in collaborative environments.
• Must have a valid Oregon driver’s license and willing to travel as needed

Responsibilities

• Conduct comprehensive nutritional assessments, including detailed health histories, symptom analysis, and current lab assessment ordered and interpreted by a Medical Provider.
• Develop and implement individualized functional nutrition plans in collaboration with the medical provider, incorporating therapeutic diets (e.g., anti-inflammatory, low-FODMAP, ketogenic), supplementation, and behavioral strategies.
• Collaborate with medical providers to co-manage patient care, providing nutrition recommendations that complement medical treatments and monitor progress through follow-up visits.
• Educate patients on functional nutrition principles, empowering them with tools for sustainable lifestyle changes via in-person, virtual, or group sessions as part of an integrated care model.
• Document patient encounters, care plans, and outcomes in electronic health records (EHR) systems in alignment with provider directives, ensuring compliance with HIPAA and clinic protocols.
• Maintain current knowledge of emerging research in functional nutrition and integrative health through continuing education ensuring alignment with clinical best practices and organizational standards.
• Contribute to the development of training and educational programs for the Functional Health team
• Participates in delivering training sessions
• Assist with clinic operations and patient care activities as needed, within training competency and scope of practice
