CVS Health, through Signify Health, is seeking a Full Time Physician to provide in-home health evaluations across assigned coverage areas. The role involves detailed patient assessments, history review, and a physical examination conducted in patients’ homes.

Travel is extensive, up to 100% across defined regions. Candidates must hold active MD/DO licenses and be able to obtain multi-state licenses as required. A home-based, patient-centered approach is essential for this position.

Qualifications

• Active, unrestricted MD/DO licenses in coverage areas
• Ability to obtain and maintain multi-state licenses as required

Responsibilities

• Provide In-Home Health Evaluations
• Travel up to 100% within defined coverage area
• Perform medication and medical history review and physical evaluation
• Utilize iPad-based clinical workflow during evaluations
• May conduct virtual visits as needed
• Provide Diagnostic & Preventive Services as indicated
