Job summary

Concept Rehab is seeking a Full-Time Speech-Language Pathologist (SLP) in Port Huron, MI to provide speech, language, cognitive, and swallowing therapy in a skilled nursing facility.

You will evaluate patients, develop individualized plans, collaborate with nursing and therapy teams, and document progress while supporting patient and family education.

Qualifications

• Current Michigan SLP license.
• ASHA CCC-SLP preferred.
• Master's degree in Speech-Language Pathology.
• Experience in SNF, rehabilitation, or geriatric care is preferred.
• Strong communication, organization, and clinical skills.

Responsibilities

• Evaluate patients and develop individualized treatment plans.
• Provide speech, language, cognitive, and swallowing therapy.
• Monitor patient progress and adjust treatment plans as needed.
• Collaborate with nursing, therapy, and interdisciplinary teams.
• Complete accurate and timely clinical documentation.
