Concept Rehab, Inc. is seeking a compassionate Full-Time Speech-Language Pathologist (SLP) to join our rehabilitation team in Okemos, Michigan. You will evaluate patients and develop individualized treatment plans to help improve communication, swallowing, and overall quality of life.

This SNF setting emphasizes collaboration with physicians, nursing staff, and other therapy colleagues. A Master’s in Speech-Language Pathology and Michigan licensure are required; CCC-SLP is preferred, CF

Qualifications

• Master’s degree in Speech-Language Pathology from an accredited program.
• Current Michigan SLP licensure or eligibility to obtain licensure.
• CCC-SLP preferred; CF candidates may be considered.
• Experience in SNF or post-acute rehabilitation settings preferred.
• Strong communication, organizational, and clinical documentation skills.

Responsibilities

• Evaluate patients and develop individualized speech therapy treatment plans.
• Provide speech-language therapy services to patients with communication, cognitive, and swallowing disorders.
• Assess and treat dysphagia and other speech-related conditions as appropriate.
• Monitor patient progress and adjust treatment plans based on individual needs.
• Complete accurate and timely clinical documentation in accordance with facility and regulatory requirements.
• Collaborate with physicians, nursing staff, occupational therapists, physical therapists, and other healthcare professionals.
• Educate patients, families, and caregivers on treatment plans and strategies.
• Participate in care planning and interdisciplinary team meetings as needed.
