Job summary

Concept Rehab is seeking a compassionate and motivated Full-Time Speech-Language Pathologist (SLP) to join our rehabilitation team in Okemos, Michigan. You will evaluate patients, develop individualized treatment plans, and provide therapy to address communication, cognitive, and swallowing disorders in a SNF setting.

Ideal candidates hold a Master’s degree in Speech-Language Pathology and a Michigan license; CCC-SLP is preferred. CF candidates may be considered.

Qualifications

• Master’s degree in Speech-Language Pathology from an accredited program.
• Current Michigan Speech-Language Pathologist license or eligibility to obtain licensure.
• CCC-SLP preferred; CF candidates may be considered if eligible.
• Experience in a skilled nursing facility or post-acute rehabilitation setting preferred.

Responsibilities

• Evaluate patients and develop individualized speech therapy treatment plans.
• Provide speech-language therapy services to patients with communication, cognitive, and swallowing disorders.
• Assess and treat dysphagia and other speech-related conditions as appropriate.
• Monitor patient progress and adjust treatment plans based on individual needs.
• Complete accurate and timely clinical documentation in accordance with facility and regulatory requirements.
• Collaborate with physicians, nursing staff, occupational therapists, physical therapists, and other healthcare professionals.
• Educate patients, families, and caregivers on treatment plans and strategies.
• Participate in care planning and interdisciplinary team meetings as needed.
