Benedictine Living is seeking a Speech Language Pathologist responsible for evaluating residents, conducting ongoing assessments, planning and implementing therapy interventions to support communication and swallowing goals for diverse patient populations.
You will develop and revise plans of care, establish functional goals and discharge plans, coordinate with the care team, and educate residents, families, and caregivers on the care plan to promote optimal outcomes.
The Speech Language Pathologist is responsible and accountable for residents' evaluation, ongoing assessment, planning and implementation of therapy interventions.
Develops and revises plan of care based on the residents' needs and establishes functional goals, outcomes, and discharge plans. Plans for residents' discharge and anticipates for long term and/or short term residents' needs. Educates other health care team members, residents, family, and caregivers on information regarding the care plan for the residents.
