Job summary

LTC Therapy Services is seeking a licensed Speech-Language Pathologist (SLP) to join the rehabilitation team at Ingraham Manor Nursing Home in Bristol, CT. This full-time position offers benefits and a diverse caseload addressing communication, cognition, and swallowing needs.

You will collaborate with an interdisciplinary team to evaluate residents, create plans of care, and monitor progress while maintaining thorough documentation and pursuing ongoing professional development.

Qualifications

• Active Connecticut Speech-Language Pathologist license required.
• Graduate degree in Speech-Language Pathology from an accredited program.
• Strong clinical, communication, and organizational skills.
• New graduates are encouraged to apply.
• Skilled nursing or post-acute experience preferred, but not required.

Responsibilities

• Evaluate residents with speech, language, cognitive-communication, and swallowing disorders.
• Develop individualized plans of care based on clinical findings and resident goals.
• Provide skilled treatment addressing communication, cognition, and dysphagia.
• Monitor outcomes and modify treatment approaches based on resident progress.
• Educate residents, families, and caregivers on therapeutic strategies.
• Maintain thorough and timely clinical documentation.
• Coordinate care with PT, OT, nursing, physicians, and other healthcare professionals.
