Job summary

Maplewood of Sauk Prairie is seeking an RN for a full-time PM/2nd shift with a $7,000 sign-on bonus. The RN will deliver direct clinical care, manage medications and care plans, and supervise CNAs to ensure resident safety and quality of care. Alternating weekends required.

Responsibilities include comprehensive assessments, interdisciplinary care planning, medication administration, and supervision of nursing staff. Wisconsin RN licensure and ADN/BSN are required, with BLS/CPR certification.

Qualifications

• Assess residents through initial and ongoing physical, mental, and cognitive evaluations and monitor vital signs, skin integrity, and mobility.

• Administer oral, injectable, and IV medications and perform clinical procedures.
• Supervise LPNs and CNAs to ensure safety and regulatory compliance.

Job description

FT (PM/ 2nd Shift) $7,000 SIGN ON BONUS Maplewood is currently interviewing RNs that can contribute to a healing environment focused on the physical and emotional well-being of all our residents. At Maplewood an RN delivers direct clinical medical care, manages complex medication regimens, develops patient care plans, and supervises certified nursing assistants (CNAs) to ensure resident safety and health. Alternating weekends required.

Key Responsibilities

• Patient Assessment: Perform initial and ongoing physical, mental, and cognitive evaluations, tracking changes in vital signs, skin integrity, and mobility.
• Care Planning: Collaborate with interdisciplinary teams—including physicians, physical therapists, and social workers—to design and update personalized resident treatment plans.
• Medication & Treatment: Administer oral, injectable, and IV medications, alongside clinical procedures such as wound care and infection control.
• Staff Supervision: Direct and oversee shift activities of LPNs and CNAs, ensuring facility policies and regulatory standards are met.

Qualifications & Requirements

• Licensure: Active, unencumbered Registered Nurse (RN) license in the State of Wisconsin.
• Education: Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN).
• Certifications: Basic Life Support (BLS) or CPR certification
• Skills: Critical thinking skills, crisis management, multi-task oriented, and knowledge of long-term care regulations. Strong communication, patience, and basic computer skills for charting.
