• Delivers patient care utilizing standardized evidenced-based guidelines of care and appropriate resources, including personnel, services, supplies and equipment.
• Monitor and report changes in patient symptoms or behavior, administer medications and document all encounters and progress notes.
• Communicate with collaborating physicians or specialists regarding patient care.
• Educates patients about health maintenance and disease prevention.
• Educates care plans, provides emotional support and advice to patients and their family members
