McGregor PACE is expanding and hiring a Registered Dietitian to join our community-based care team serving frail and elderly clients across Ohio. The role focuses on direct participant care, nutrition plan development, and inter-disciplinary collaboration with a strong emphasis on education for participants, families, and staff.

The position supports home visits and clinic activities, with flexibility for remote work.

Qualifications

• Registered Dietitian licensed in Ohio.
• Bachelor’s degree in nutrition and dietetics; master’s degree preferred.
• Minimum of one year experience with frail/elderly population.
• Experience in community-based programs and interdisciplinary care settings desirable.
• Valid Ohio driver’s license and reliable transportation.

Responsibilities

• Provide direct participant care and nutrition care plans for enrollees.
• Develop and implement nutritional care plans and follow-up systems.
• Educate participants, families and staff as needed on nutrition and care.
• Maintain thorough documentation and coordinate with the Interdisciplinary Team.
• Support care plans to meet each participant’s needs.

McGregor PACE is an innovative, community-based Program of All-inclusive Care for the Elderly. Our five senior day centers and clinics are located in the counties of Cuyahoga, Lorain, and Summit.

McGregor PACE is expanding! We have opportunities available for an experienced and compassionate Registered Dietitian to work in a community-based setting with the frail and elderly populations.

McGregor PACE takes great pride in the provision of comprehensive services for frail older persons. The Dietitian provides direct participant care to program enrollees; develops and implements nutritional care plans, communicates plans, provides education to participants, family, and staff as needed, establishes a follow-up system and maintains documentation. The Dietitian works with an Interdisciplinary Team to develop plans of care that meet the participants' needs.

The workplace setting includes our PACE senior day centers and clinics, and home visits which may include visiting assisted living facilities, long-term care facilities, group homes, and community homes. There is also flexibility for remote work!

Qualifications include:

• Registered Dietitian; licensed as a clinical dietitian in the state of Ohio
• Bachelor’s degree in nutrition and dietetics, required; master’s degree, preferred
• Minimum of one year’s experience working with a frail or elderly population, required
• Experience in community-based programs and interdisciplinary care settings, desirable
• Current Ohio driver's license and reliable transportation, required
