Job summary

Renewal Rehab is seeking a Licensed Physical Therapist Assistant for a full-time position in Traverse City, MI within a SNF/Long-Term Care setting. Must have an active PTA license to practice in Michigan and be prepared to collaborate with a supervising PT.

Qualifications

Responsibilities

• Provide physical therapy treatments under supervision of a licensed Physical Therapist, following state practice guidelines.
• Assist in identifying appropriate patients for therapy and coordinate with the PT for evaluation and referral.
• Collaborate with team members to ensure quality outcomes.
• Maintain documentation to meet all regulatory requirements.
• Ensure all required physician orders are obtained and up to date.
