Ultimate Therapy is seeking a full-time Physical Therapist Assistant (PTA) to work at Autumn Lake at Windsor in Windsor, CT. You will deliver individualized treatment under the supervising Physical Therapist in a skilled nursing rehabilitation setting, with a focus on functional mobility and safe transfers.

Qualifications

• CT PTA license
• Ability to follow established care plans and resident-specific precautions
• Clear communication, sound observation, and accurate documentation
• New graduates welcome to apply; skilled nursing or post-acute experience a plus

Responsibilities

• Carry out prescribed interventions addressing residents' functional mobility goals.
• Coach residents through exercises for strength, balance, and activity tolerance.
• Assist with walking, bed mobility, and transfers with appropriate support.
• Reinforce techniques for mobility devices and safety.
• Observe progress and communicate findings to the supervising PT.
• Document care promptly and collaborate with nursing and therapy colleagues.
