Job summary

Ultimate Therapy seeks a full-time Physical Therapist Assistant (PTA) to provide personalized mobility-focused care at Autumn Lake in Windsor, CT. You will work under the supervising Physical Therapist to help residents improve daily movement in a skilled nursing setting.

Responsibilities include implementing interventions, guiding exercises, assisting with transfers, and documenting progress while coordinating with nursing and therapy staff. New graduates are welcome to apply.

Qualifications

• Connecticut PTA license is required.
• Ability to follow established care plans and resident-specific precautions.
• Clear communication, good observation, and accurate documentation.

Ultimate Therapy is seeking a full-time Physical Therapist Assistant at Autumn Lake at Windsor to deliver individualized treatment under the supervising Physical Therapist's direction in a skilled nursing rehabilitation setting.

What You'll Do

• Carry out prescribed interventions that address residents' functional mobility goals.
• Coach residents through exercises for strength, balance, and activity tolerance.
• Practice walking, getting in and out of bed, and transferring between surfaces with appropriate assistance.
• Reinforce techniques for using prescribed mobility devices safely.
• Observe progress, symptoms, and treatment responses, communicating relevant findings to the supervising PT.
• Document care promptly and partner with nursing and therapy colleagues to support consistent mobility routines.
