Job summary

Evolve Therapy Services is seeking a dedicated Physical Therapist Assistant for an in-house Skilled Nursing Facility in Milwaukee, WI. The role supports a Physical Therapist to deliver comprehensive therapy services to residents in a team-based setting.

Responsibilities include conducting screenings, assisting with treatment plans, and providing individual and group therapy focused on mobility, strength, balance, and functional independence.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education and professional development to maintain licensure and certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
• Assist the Physical Therapist in identifying goals and treatment plans.
• Conduct individual and group treatment as outlined in the treatment plan, including mobility, gait, range of motion, and modalities.
• Assist in selecting therapy techniques/media and adapt as indicated by residents.
• Monitor residents’ response to intervention and modify as needed in consultation with the Physical Therapist.
• Establish and modify resident schedules in collaboration with the Physical Therapist and team.
• Train residents, families, and staff in equipment use and safety.
• Participate in resident, family, and staff education.
• Ensure compliant and medically necessary documentation in NetHealth EMR systems.
