Job summary

Evolve Therapy Services is seeking a compassionate Physical Therapist Assistant for an in-house Skilled Nursing Facility in Milwaukee, WI. The PTA will provide supervised therapy, assist in treatment planning, and support residents and staff in achieving therapeutic goals.

Responsibilities include screenings, interval evaluations, patient and family education, and accurate CMS documentation in NetHealth EMR.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Ongoing continuing education to maintain licensure/certification.

Responsibilities

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention/treatment.
• Assist the Physical Therapist in evaluations, goal setting, and treatment planning.
• Assist in establishing and modifying individual resident schedules with the team.
• Administer treatment programs including mobility, gait, strength, and range of motion activities.
• Train residents, families, and staff in use or application of equipment.
• Document therapy activities and CMS-compliant records in NetHealth EMR.
• Contribute to quality assurance activities and maintain productive schedules.
