Job summary

Premise Health Inc. in Modesto, CA, seeks a Full Time Physician to deliver occupational health and primary care to client employees and dependents aged 2 and older at the EJ Gallo Health Center.

Hours are Monday–Friday, 8 AM–5 PM, with no call and no weekend shifts. You will provide episodic, preventive, and acute care, perform physicals, manage treatment plans, referrals, and medical records, and collaborate with the medical leadership to meet health center goals while upholding Premise Health

Qualifications

• State licenses to practice.
• Federal DEA license (matching each state of practice/oversight).
• Current board certificate (ABMS, AMA or AOA).
• Medical Degree.
• CPR (BLS for the Healthcare Provider/Professional Rescuer or ACLS) certification with hands-on evaluation.
• At least three years of post-residency clinical experience.
• Experience in primary care and occupational medicine in outpatient/urgent care/ER settings.

Responsibilities

• Provides episodic, preventive, and acute health care to patients.
• Provides occupational health services (post-offer physicals, surveillance exams, medical monitoring, return-to-work exams, workers' compensation).
• Conducts histories and physical examinations and prescribes medications and treatments.
• Develops treatment plans in collaboration with patients and educates on medical issues.
• Provides referrals internally and externally to medical resources or specialties.
• Evaluates effectiveness of care and monitors patient progress.
• Identifies emergencies and provides care within scope of practice and privileging.
• Maintains medical records using EMR software.
• May formulate and administer health programs as needed.
• Assumes responsibility for care in the absence of Medical Leader.
