Job summary

CareMed Health Solutions is seeking a dedicated NP to provide primary and preventive care for residents in skilled nursing facilities. The role includes managing acute changes, planning advanced care, and prescribing medications per Ohio law, with collaboration across an interdisciplinary team.

Requirements include an Ohio NP license, board certification, DEA licensure, and experience in long-term care with EMR proficiency; ability to coordinate with primary care physicians is essential.

Qualifications

• Current, unrestricted NP license in Ohio.
• Board Certification as NP.
• DEA licensure to prescribe controlled substances.
• Experience in a long-term care setting.
• Ability to work with an EMR system.
• Ability to manage patients with advanced illness and multiple chronic conditions.
• Experience in an interdisciplinary care team.

Responsibilities

• Provide primary and preventative care for long term care patients residing in skilled nursing facilities.
• Manage Residents with acute change in condition.
• Advanced care planning and chronic care management.
• Prescribe appropriate medications, including controlled substances in accordance with OH Law.
• Order, review and/or follow-up with treatments and diagnostic testing.
• Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.
• Collaborate with primary care physician.

• Ability to manage patients with advanced illness and multiple chronic conditions.
• Ability to work within an interdisciplinary care team to coordinate patient care.
• Psych experience a plus
