Job summary

CareMed Health Solutions is seeking a Nurse Practitioner to provide primary and preventive care to residents in long-term care facilities. You will manage patients with acute changes, engage in advanced care planning and chronic disease management, and prescribe medications in accordance with Nebraska law.

The role requires Nebraska NP licensure, board certification, DEA licensure, and experience in geriatric or adult care.

Qualifications

• Current, unrestricted NP license in Nebraska.
• Board Certification in a relevant NP specialty.
• DEA licensure required.
• Clinical background in adult, family or geriatric care.

Responsibilities

• Provide primary and preventive care for long-term care patients.
• Manage residents with acute change in condition.
• Advanced care planning and chronic care management.
• Prescribe medications, including controlled substances per Nebraska law.
• Order, review and follow-up with treatments and diagnostics.
• Communicate with nursing home staff regularly about patient changes.
• Collaborate with primary care physician.

We are currently seeking Nurse Practitioners provide medical services to patients living in long term care facilities.

NP ROLE AND RESPONSIBILITIES

• Provide primary and preventative care for long term care patients residing in skilled nursing facilities.
• Manage Residents with acute change in condition.
• Advanced care planning and chronic care management.
• Prescribe appropriate medications, including controlled substances in accordance with Nebraska Law.
• Order, review and/or follow-up with treatments and diagnostic testing.
• Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.
• and/or the care plan.
• Collaborate with primary care physician.

Np Qualifications And Education Requirements

• Current, unrestricted NP license in the state of Nebraska.
• Board Certification.
• DEA licensure.
• Clinical background in adult, family or geriatric specialties.
• Experience working in a long-term care setting.
• Ability to work with an EMR.
• Ability to manage patients with advanced illness and multiple chronic conditions.
• Ability to work within an interdisciplinary care team to coordinate patient care.
• Psych experience a plus
