Job summary

CareMed Health Solutions is seeking a Nurse Practitioner to provide medical services to residents in skilled nursing facilities in Iowa.

As part of an interdisciplinary team, you will deliver primary and preventive care, manage acute changes, coordinate care planning, and work with staff to optimize treatment plans.

You will prescribe medications within Iowa law, order tests, and collaborate with primary care physicians to ensure high-quality, person-centered care.

Qualifications

• Current, unrestricted NP license in the state of Iowa.
• Board Certification as an NP.
• DEA licensure and authority to prescribe.
• Clinical background in adult, family or geriatric specialties.
• Experience in long-term care settings.
• Ability to work with an EMR.
• Ability to manage patients with advanced illness and multiple chronic conditions.
• Ability to work within an interdisciplinary care team to coordinate patient care.
• Psych experience is a plus.

Responsibilities

• Provide primary and preventative care for long term care residents.
• Manage residents with acute changes in condition.
• Engage in advanced care planning and chronic care management.
• Prescribe appropriate medications, including controlled substances in accordance with Iowa law.
• Order, review and follow up with treatments and diagnostic testing.
• Communicate with nursing home staff regarding changes in condition and care plans.
• Collaborate with primary care physicians.
