Job summary

CareMed Health Solutions is seeking a Nurse Practitioner to provide primary and preventive care for residents in long-term care facilities in Alabama. You will manage acute changes, advance care planning, and coordinate with the interdisciplinary team to ensure high-quality patient outcomes.

The role requires AL NP licensure, board certification, DEA licensure, and experience in geriatric/long-term care settings, with EMR proficiency. Collaboration with primary care physicians is essential.

Qualifications

• Must hold current, unrestricted NP license in Alabama.
• Board certification required.
• DEA licensure required.
• Clinical background in adult, family or geriatric specialties.
• Experience working in a long-term care setting.
• Ability to work with an EMR.
• Ability to manage patients with advanced illness and multiple chronic conditions.
• Ability to work within an interdisciplinary care team.
• Psych experience a plus

Responsibilities

• Provide primary and preventative care for long term care patients residing in skilled nursing facilities.
• Manage Residents with acute change in condition.
• Advanced care planning and chronic care management.
• Prescribe appropriate medications, including controlled substances in accordance with AL Law.
• Order, review and/or follow-up with treatments and diagnostic testing.
• Communicate with the nursing home staff on a regular and as needed to discuss changes in condition.
• Collaborate with primary care physician.
